Make a Referral

We accept referrals for specialist behaviour support across Victoria. Referrals can be made by support coordinators, families and carers, allied health professionals, and service providers.

We work with individuals with complex behavioural, disability, and trauma-related needs, particularly where there are concerns around risk, escalation, or inconsistent support environments.

1.Who We Support

Referrals are appropriate where:

  • There are behaviours of concern impacting safety or quality of life

  • Support teams are experiencing inconsistency or difficulty implementing strategies

  • Risk has increased or previous supports have not been effective

  • There is a need for functional assessment, behaviour support planning, or implementation support

We support people across:

  • Home and community settings

  • Supported accommodation (SIL/SDA)

  • School and day program environments

2.What to Include in a Referral

To support timely triage, please include:

  • Participant name, age, and location

  • NDIS status and plan details (if applicable)

  • Summary of current concerns / behaviours of concern

  • Any known risks (to self or others)

  • Current supports and providers involved

  • Relevant reports or assessments (if available)

3.What Happens Next

  1. Referral is received and reviewed

  2. Initial contact is made within 2 business days

  3. A brief discussion is held to confirm suitability and scope

  4. Intake process and service agreement (if appropriate)

  5. Service commencement based on availability

Please note:

  • Acceptance of referral is subject to scope, capacity, and risk assessment

  • This is not a crisis or emergency service

  • If there is immediate risk, contact emergency services or appropriate crisis supports
